Phoenix Medical Centre provides compassionate, community-based outreach services for individuals and families in Mississauga, Oakville and surrounding communities. Our outreach programs are designed to help people access healthcare, addiction and mental health support, housing resources, emergency assistance and in-home care when traditional healthcare settings may not be enough.
Through addiction and mental health outreach, homeless outreach and emergency support, senior outreach and care at home, our team works to reduce barriers and connect individuals with appropriate services based on their needs. Phoenix Medical Centre also works alongside trusted community organizations, including SHIP (Services and Housing in the Province), PAARC (Peel Addiction Assessment and Referral Centre), and The Salvation Army serving Hamilton, Halton and Brantford, to help provide coordinated support and access to additional community resources.
